The Fidium Fiber Advocate Referral Program ("Program") is open to pre-qualified community members who refer new small business leads to Fidium Fiber. To be eligible, you must submit referrals through the designated form on the Fidium Fiber website. A referral must be a new small business lead that has not been previously contacted by Fidium Fiber within the past 12 months. For each qualified referral that results in a signed contract, the referrer will received a $100 gift card and referee will receive a $50 gift card. Gift cards will be issued electronically after install is complete and 90 days of active service. In accordance with IRS regulations, a W-9 form must be completed and submitted by any business or individual who receives total rewards exceeding $599 in a calendar year through this referral program. This includes, but is not limited to, gift cards and other incentives provided as rewards for referrals. All referrals must be made in good faith and in accordance with applicable laws and regulations. Fidium Fiber reserves the right to modify or terminate the Program at any time without prior notice. Participation in the Program constitutes acceptance of these terms.
